BACKGROUND
Oncocytic neoplasms of the salivary glands are rare and the differential diagnosis between oncocytic carcinomas (OCs) and oncocytomas is difficult. We present 5 cases of oncocytoma and 3 cases of OC of the salivary glands with clinicopathological and immunohistochemical comparisons.
METHODS
Eight cases of oncocytic neoplasms diagnosed at Asan Medical Center between 1998 and 2009 were reviewed for clinical data and histological features. Immunohistochemical staining for epidermal growth factor receptor (EGFR), human epidermal growth factor receptor 2 (Her-2), c-kit, p53, and Ki-67 was done.
RESULTS
Cytological differences between oncocytomas and OCs were not obvious, but unequivocal infiltrative growths were identified in 3 cases, rendering the diagnosis of oncocytic carcinoma. When the remaining cases were classified as oncocytomas, there was no difference in age, size, and clinical symptoms between oncocytomas and OCs. Two of 3 OCs showed strong membranous expression of c-kit, but all oncocytomas were negative. The proportion of p53-positive cells was larger in OCs than oncocytomas. Her-2 or EGFR expression was absent, and Ki-67 labeling indices were less than 1% in all cases.
CONCLUSIONS
An infiltrative growth pattern, strong membranous expression of c-kit, and an increased proportion of p53-positive cells are features that can differentiate OCs from oncocytomas of the salivary glands.

Abstract PDF: A rare case of functioning oncocytic parathyroid adenoma associated with primary hyperparathyroidism was found in a 45-year-old woman. The preoperative serum calcium level was 13.1 mg/dL, the phosphate level was 2.44 mg/dL and the parathyroid hormone level was 153 pg/mL. Neck CT revealed a 2.5x1x1 cm, well enhanced mass behind the left thyroid gland, which was compatible with parathyroid adenoma. The removed parathyroid gland showed a well circumscribed, ovoid, brown colored, soft, solid mass. Histologically, this mass was composed of broad sheets of uniform cells having round dense nuclei and abundant eosinophilic granular cytoplasms. The adjacent rim of the normal parathyroid tissue was identified in the periphery of the mass. After operation, all hormone levels were normalized.

Abstract PDF: Oxyphilic (H rthle) cells have abundant eosinophilic granular cytoplasms and occur in both benign and malignant neoplasms of the thyroid gland. Most published studies described mainly oxyphilic tumors of follicular type, and reports on oxyphilic papillary thyroid carcinomas are rare. The oxyphilic variant of papillary carcinoma differs from the classic papillary carcinoma in its more aggressive biological behavior. We report a case of oxyphilic papillary carcinoma exhibiting exclusively oxyphilic cells in a follicular pattern without papillary structures. The pattern of papillary carcinoma was confirmed by fine needle aspiration material and lymph nodal metastasis, both of which revealed typicaled papillary structures with characteristic nuclear features, psammoma bodies, and oxyphilic cytoplasm.

Abstract PDF: Lymphoid infiltration can be seen in some lesions such as Hashimoto's thyroiditis, subacute thyroiditis and several neoplasm of the thyroid. In case of malignancy, there are a few reports of lymphoid infiltration in the diffuse sclerosing variant of papillary carcinoma. But heavy lymphoid infiltraton without evidence of sclerosis is uncommon. We experienced a case of papillary oxyphilic carcinoma with massive lymphoid infiltration, which looks like Warthin tumor of salivary gland. However cytological feature of epithelial cells exhibit that of papillary carcinoma.

Abstract PDF: Oncocytic glomus tumor is a newly recognized, rare variant of glomus tumor. In this study, we describe a case of oncocytic glomus tumor occurring in the nail bed of the finger. A 25-year-old woman presented with a tiny and painful mass on her hand, and she had had this lesion for six years. Upon microscopic examination, there were oval to polygonal tumor cells characterized by plump eosinophilic granular cytoplasm, which were arranged in sheets around the thin vessels. Immunohistochemically, the tumor cells were strongly reactive for smooth muscle actin, vimentin, and negative for S-100 protein, alpha-fetoprotein, HMB-45, desmin, CD34, and pancytokeratin. The diagnosis was a glomus tumor of the oncocytic variant.

Abstract PDF: Oxyphilic clear cell carcinoma of the ovary is a variant of clear cell carcinoma with abundant eosinophilic cytoplasm described by Young & Scully in 1987. Thorough samplin is needed to identify typical foci of clear cell carcinoma for the differential diagnoses from a variety of ovarian tumors with oxyphilic cells. We report a case of oxyphilic clear cell carcinoma in a 65-year-old female patient who presented with vaginal spotting and lower abdominal discomfort. The excised mass was a 10x8x7cm sized, well circumscribe yellowish white solid ovarian tumor. Microscopically, the tumor showed glandular, papillary and alveolar growth patterns composed of cuboidal or hobnail-shaped oxyphilic cells.
